Such simple as SD800
Does anyone have a link for that?
Also Dual-mono (integrated pot.) version might be usable..

Which values in milliseconds you have measured when setting your Sub(s) in sync with mains?

Check ReD_MuZe's Phase Fix. The page is a bit garbled, but you can find the file's url and it's still there.

It's not specifically for a sub, but you can use it Mono as 0-200 samples delay on the sub, or as stereo on the stereo monitor - whichever is closer to you. To know the amount of samples to delay, sound travels at 1ft/ms...

I edited this and added some simple math:
@ 44.1kHz, 44100 samples per second.
-> 1ms or 1ft equals 44 samples
-> speakers 3ft or 1m closer can be delayed about 3x44 or ~ 130samples.

@ 96kHz, 1ft ~ 96 samples
-> 1m ~ 290 samples
