when connecting a 3 connector S/TDM cable between two cards, is there a preference for using the middle connector or the one at the end?

the first cable goes to the connector closest to the back of the computer(i/o). there is a map in the manual...

I mean the first connector, not the first cable.
You have three connectors on a cable, and two cards. You plug the one on the beginning of the cable onto the first card. Will you use the second, or the third connector on that cable for the other card, leaving the other connector unconnected?
That's not explained in the manual, unless I'm overlooking something.

2 cards? connect the outside connectors and leave the middle one disconnected.

As Gary says, ideally use the end-of-cable connectors.
It's all about reflections of the eletrical wave propagations in the cable etc.
While the STDM buss signal way is fairly short, the optimal way is to use the ends.
As with SCSI of the day (way back before SATA took over eveyrthing), you just might get spurious signals if you have extra length cables that are not connected (or un-terminated - I don't know if this is the case with the SDTM buss).
But practically, I've found that there is no real differenec - most cables have 3-8 cm between connectors, and you probably won't feel any difference between what connetors you're using.
